Steps to create health id card:

Login to : https://healthid.ndhm.gov.in/register

Enter Aadhar no / mobile no

Followed by OTP received on the aadhar linked mobile                 number

Check your details - name, date of birth, address etc.

You can change the profile picture to a more recent photo if you want. Once you submit - an ID card like the one below is created - you can download that as an image.
